What Is the Free-Market Approach to Health Care Reform?
President Obama is right when he says that the U.S. health care system needs reform. Although this country provides the finest care in the world, our health care system has serious problems. It costs too much. Too many people lack health insurance. And quality can be uneven.
But a government takeover of the health care system, as proposed by the president and some in Congress, would be a step in the wrong direction. Instead, we should pursue a uniquely American solution, one that builds on free markets, competition and choice.
1. Let individuals control their health care dollars, and free them to choose from a wide variety of health plans and providers. 2. Move away from a health care system dominated by employer-provided health insurance. Health insurance should be personal and portable, controlled by individuals themselves rather than government or an employer. Employment-based insurance hides much of the true cost of health care to consumers, thereby encouraging over-consumption. It also limits consumer choice, since employers get final say over what type of insurance a worker will receive. It means people who don’t receive insurance through work are put at a significant and costly disadvantage. And, of course, it means that if you lose your job, you are likely to end up uninsured as well. 3. Changing from employer to individual insurance requires changing the tax treatment of health insurance. The current system excludes the value of employer-provided insurance from a worker’s taxable income. However, a worker purchasing health insurance on their own must do so with after-tax dollars. This provides a significant tilt towards employer-provided insurance, which should be reversed. Workers should receive a standard deduction, a tax credit, or, better still, large Health Savings Accounts (HSAs) for the purchase of health insurance, regardless of whether they receive it through their job or purchase it on their own. 4. We need to increase competition among both insurers and health providers. People should be allowed to purchase health insurance across state lines. One study estimated that that adjustment alone could cover 17 million uninsured Americans without costing taxpayers a dime. 5. We also need to rethink medical licensing laws to encourage greater competition among providers. Nurse practitioners, physician assistants, midwives, and other non-physician practitioners should have far greater ability to treat patients. Doctors and other health professionals should be able to take their licenses from state to state. We should also be encouraging innovations in delivery such as medical clinics in retail outlets. 6. Congress should give Medicare enrollees a voucher, let them choose any health plan on the market, and let them keep the savings if they choose an economical plan. Medicare could even give larger vouchers to the poor and sick to ensure they could afford coverage. 7. The expansion of “health status insurance” would protect many of those with preexisting conditions. States may also wish to experiment with high risk pools to ensure coverage for those with high cost medical conditions.

Progressives will not win the public option argument until we make the sell to the majority of citizens who have healthcare through their employers. As long as the majority of voters think that it is not their problem they will continue to view healthcare as some form of welfare! We need to point out the inadequacies of the private insurance plans and demonstrate their vulnerability. I will bet it all that 9/10 of those covered are unaware of the details of their policies. All private plans impose limits on various services from hospital stay, to types of drugs, to home care, to procedural options. Even the so called “Cadillac” plans stop short if the bill gets too high. We have numerous cases of people with insurance being dropped for xyz or being laid off and finding themselves unable to afford insurance. We have to set a vision and provide details based on how the public plan will work (although it may be altered during the legislation process). Proponents have to identify what people are getting for their money. Funding will become less of an issue when the public knows the value-add. If there is a clear vision, resistance to creative financing will dissolve. People can weigh value versus potential cost. The argument for funding can be won whether it’s from soda or obesity taxes, to a change in Medicare tax from flat to progressive, to charging higher premiums. The public plan at its core should promise that once you pay your premium, mandated or not, that you are covered and aside from insignificant co-pay, you will not receive any other medical bill. The public plan will be priced based on income and it’s OK if it’s affordable but not cheap. What I mean is that the Public plan will be the higher end product. We know that the public plan should be independent of your employment or employment status. Let’s flush out the other details, not leave it to Baucus and Grassley. Until we define what the public plan entails, it is susceptible to misinformation. It’s hard to argue with a plan that will not turn you down for a preexisting condition. It is hard to argue with a plan that will not cap your stay at a hospital based on a dollar threshold. It is hard to argue with a plan that allows Doctors to prescribe medicine based on their discretion. No one has defined the public plan. And until it’s done the public plan is just this monster in the closet.

The minister known as Reverend Ike, who preached the gospel of material prosperity to millions nationwide, has died. He was 74.
Family spokesman Bishop E. Bernard Jordan told The New York Times that The Rev. Frederick J. Eikerenkoetter II, who suffered a stroke in 2007 and never fully recovered, died in Los Angeles. He moved to the city two years ago, Jordan said.
Reverend Ike preached the power of what he called "positive self-image psychology" to his 5,000 parishioners at the United Church Science of Living Institute. The church was housed in a former movie theater in New York's Washington Heights neighborhood.
In the 1970s, Reverend Ike was one of the first evangelists to reach an audience of millions through television.
"This is the do-it-yourself church," he proclaimed. "The only savior in this philosophy is God in you."
Reverend Ike stretched Christian tenets, relocating the idea of God to the interior of the self, with the power to bring the believer anything he or she desired in the way of health, wealth and peace of mind.
The philosophy did not sit well with traditional Christian ministers and civil rights leaders who felt black churches should focus on social reform rather than self-fulfillment.
His critics said he preyed on the poor and conned the faithful into giving him donations that he spent on cars, clothes and homes for himself. The IRS and the Postal Service investigated his businesses.
Others defended his philosophy of mind over matter, which appealed to middle-class believers who felt their hard work should be rewarded in this life.

Today, The New York Times published a new poll that it erroneously claimed found "growing unease" about health care reform.
The actual numbers were buried in the article and actually seemed to point to broad support for health care reform.
The article, entitled, "New Plan Finds Growing Unease on Health Plan." The thrust of the headline was consistent with a series of articles the Times has published framing opposition to health care overhaul as "growing."
However, while the Times typically prints the poll findings on the front page with multli-media graphics to accompany the findings, this article buried the actual poll numbers supporting their assertion that there is "growing unease on a health plan" almost 20 paragraphs into the article.
Moreover, the actual poll numbers suggest that America is very enthusiastic about health reform. The article cites the worst numbers first: Obama's approval rating is down 10 points from April and 69 percent said they worried their plans would diminish in quality if the government passed a public plan option.

The latest, the details from the deal they struck with E&C Chair Henry Waxman have emerged, and they belie the claims of fiscal responsibility:
Despite Blue-Dog claims that a compromise they engineered Wednesday would trim $100 billion from the cost of health-care legislation, their weakening of the public option for insurance coverage could end up erasing those savings....
They got the headlines they wanted -- "House Democrats Trim $100B From Health Bill," The Washington Post wrote - but sources on Capitol Hill and outside of government say the changes to the public plan could actually increase the cost of the bill anywhere between $60 billion and $100 billion, wiping out all or most of the ostensible savings....
The Congressional Budget Office has not yet made an estimate of how cost-effective the revised public option would be. But The Walker Report blog, which has done detailed analysis of all variations of health care legislation, noted that the new public option is similar to that put forward by the Senate Health Education Labor and Pensions (HELP) Committee. The CBO was vague on how much the HELP Committee's public option would cost, but Walker writes that it "would not be noticeably cheaper than private insurance or save the government much money."
Politico, meanwhile, reported on Wednesday night that the savings lost by switching to the Blue Dog public plan would be $60 billion.
On the far end of cost-savings analysis is a June 2009 report issued by the Common Wealth Fund, a private foundation that conducts health care analysis and promotes a better performing health care system. Titled "Fork In The Road," the report concluded that a public option paying providers Medicare rates (which are lower than any proposal put forward to date) would produce savings for the government of roughly $3 trillion. A public plan that provided rates somewhere between Medicare and private plan rates (where the Blue Dog approach would likely be) would yield only $2 trillion in savings.
As slinkerwink and shevas01 have noted, the CPC has protested this agreement, and met with leadership to reinforce their refusal to support any bill that does not meet the criteria they've established for a public option.
Keep in mind as well that the bill E&C comes up with will be reconciled during recess with the Tri-Committee chairs, Waxman, Miller, and Rangel. With enough pressure from the larger voting blocs of the progressive and minority caucuses--who's voting power is much larger than the Blue Dogs--the Blue Dog provisions could be stripped out. As if to signal leadership's approach on this, Pelosi today blasted the insurance industry:
"It's almost immoral what they are doing," Pelosi said to reporters, referring to insurance companies. "Of course they've been immoral all along in how they have treated the people that they insure," she said, adding, "They are the villains. They have been part of the problem in a major way. They are doing everything in their power to stop a public option from happening."
Meanwhile, this is the thanks Waxman gets for negotiating with the Blue Dogs:
On Thursday, some of the Blue Dogs continued to stray, aligning with Republicans on amendment votes.
Waxman only narrowly beat back, 28-29, one GOP amendment that would have imposed new identity proof requirements on Medicaid patients seeking care. Five Blue Dogs voted for the amendment.
Waxman and other foes of the proposal said it would force people to provide proof of citizenship in the emergency room and deny them needed care.

Orlando Sentinel Tells Bill Posey To "Withdraw Ridiculous" Birther Bill
Florida Representative Bill Posey, the original author of the so-called "Birther bill" that has earned the crackpot fringe its only thin shard of legislative legitimacy, is taking heat from his hometown Orlando Sentinel. In today's editorial, the Sentinel draws a bead on Posey, and quite rightly describes his bill as "ridiculous":
If freshman Congressman Bill Posey of Rockledge doesn't want people to think he questions President Obama's citizenship, he'll have to do more than vote with the House in celebrating Hawaii's 50 years and its status as Mr. Obama's birthplace, as he did Monday.
He'll have to withdraw his ridiculous bill that compels candidates for president to produce their birth certificates -- a measure latched onto by tin-hatters who claim Mr. Obama was born in Africa and thus can't be president.
Sotomayor Creates Problems For GOP Gubernatorial Candidates
The Supreme Court nomination of a Latina judge, Sonia Sotomayor, is forcing some Republicans to weigh the benefits of appealing to Hispanic voters against the cost of alienating the conservative base.
The struggle has manifested itself most clearly among Republicans currently running for office. Gubernatorial candidates Chris Christie of New Jersey and Bob McDonnell of Virginia have found President Barack Obama's nominee for the Supreme Court a political topic best avoided.
In McDonnell's case, the Virginia Attorney General has not actually weighed in on the debate over Sotomayor's confirmation at all. "We have not been asked or put out a statement," said McDonnell spokesperson, Taylor Thornley. Asked if she could provide one, Thornley said she would "see if we can get something put together for you." Subsequent requests for that information went unreturned.
Because it has no bearing on anything that goes on in VA.
After the first debate, Democrats here were complaining that McDonnell was "nationalizing" the race; that he was putting too big a focus on federal policies that will actually have an effect on the lives of Virginians.
So-after all those complaints (and Deeds' relative lack of clarity on where he actually stood on those issues)-it's now OK to bring a "national" issue up, in the oft chance of a "macaca" moment.
That rings of insincerity and baiting (and hypocrisy). So for those who want that, look elsewhere.
And, like I said, for those who really want to know, a venue can be provided where you can ask yourself.

Which Democrats complained that he was "nationalizing" the race? This is the first I ever heard of it. Saw nothing in the local papers or on blogs and you never raised that here as an issue.
